Long Term Services and Supports

LTSS Care Partners is a team of dedicated Care Coordinators who work collaboratively with your child's healthcare providers and social support networks to improve access to care and resources. Many of our Care Coordinators are bilingual in English and other languages, ensuring effective communication for diverse families. This valuable service is provided as part of your child's MassHealth insurance coverage.

How Your Care Coordinator Can Help

Our Care Coordinators provide comprehensive support to families by:

  • Identifying your child's health and social needs
  • Coordinating with your child's doctors and social service providers
  • Supporting you with scheduling and keeping appointments
  • Assisting with transportation arrangements for medical visits
  • Arranging for in-home services when eligible
  • Supporting your child's medication and nutrition needs
  • Connecting you to free food services such as SNAP
  • Helping you understand MassHealth letters and forms
  • Connecting you to housing, rental, and utility programs and waitlists

Program Information

MassHealth has established partnerships with four non-profit organizations in the Greater Boston area to form Community Care Partners (CCP). Justice Resource Institute (JRI) is proud to be one of these partner organizations.

The LTSS Community Partners (CP) program at JRI provides care coordination services for MassHealth insured children and youth ages 3-18 (21 with exception) with medical and Social Determinant of Health (SDoH) needs.

We help people arrange LTSS and social services. We often work with children and youth that have autism, physical conditions, acquired brain injury, and intellectual & developmental disabilities.
