Huntington at Symphony

Huntington At Symphony (HAS) in Boston is a unique housing program that partners with the Boston Housing Authority and Cushing Companies to offer subsidized housing units for individuals living with HIV/AIDS (PLWHA). This program ensures residents have access to essential support services, including 24-hour concierge service and JRI Health Medical Case Management, to help them maintain their health and independence in a secure environment.

Group Adult Foster Care Services

The Group Adult Foster Care (GAFC) program at Huntington At Symphony enhances independent living for residents with HIV/AIDS or those at elevated risk of HIV who also face co-occurring psychiatric or medical disabilities. The program operates within a private property building with single-room occupancy units and provides various levels of housing and voucher-based support.

Patients have 24/7 access to concierge services and receive care from a dedicated team, including a Nurse Coordinator, Home Health Aide caregivers, and a contracted LICSW. Weekly nursing access is available, with additional consultation services provided as needed. Medicaid primarily funds the GAFC services, ensuring residents receive consistent, high-quality care tailored to their needs.

Patients may be referred through multiple sources including: housing staff, concierge staff, property management staff, other patients, loved ones (family/friends etc.), as well as outside medical staff. Most often the patients are involved in other JRI Health services and are identified as requiring additional supports, and therefore referred to GAFC. For enrollment in the program a formal referral must be placed by the patient’s primary care and reviewed by the nursing coordinator for appropriateness.
