Prevention and Access to Care and Treatment (PACT)

PACT provides comprehensive home-based care for people living with HIV and Hepatitis C who struggle to navigate healthcare systems and treatment regimens. Our Community Health Workers (CHWs) offer personalized support by assisting clients with medical appointment coordination, medication management, and in addressing various obstacles to improved health outcomes. PACT also provides training and technical assistance to organizations interested in developing their own community health worker programs.
