Housing Access Program

The Housing Access Program provides housing navigation services to individuals living with certain chronic health conditions facing housing instability.  These services include housing search, housing advocacy, linkage to legal advocacy and direct representation, and rental assistance. The Team assesses applicants' specific need(s) at intake and develops individual service plans with each client.  This may include identifying housing and community resources, applying to housing waitlists, collaborating with the Health Law Institute for legal support, and accessing financial assistance as needed with the ultimate goal of reaching housing stabilization.
