This session includes pre-medical visit planning, the role of a patient advocate in the medical and social service visits, and post-visit planning, and tools are distributed to facilitate more efficient medical visits. Communication with providers is reviewed through different case scenarios. Proper utilization of the ER, urgent care, and outpatient care is discussed in a group exercise.
