Through our office in Yarmouth Port, JRI provides services to ensure that children and their families residing on Cape Cod and the Islands, who have significant behavioral, emotional, and mental health needs obtain the services necessary for success in the home, school, and community.

In-Home Behavioral Services (IHBS)

Through JRI’s In Home Behavioral Services Program, families are provided with individualized behavioral interventions designed to manage their child’s challenging behaviors while promoting positive alternatives.  Families receive coaching and support on how to implement these interventions in their home environment.

In-Home Behavioral Services include:

  • Functional Behavioral Assessment (FBA)
  • An individualized Behavior Plan (BP)
  • Modeling and coaching of BP interventions
  • Monitoring of progress by ongoing data collection
  • Coordination and collaboration with community providers

Our treatment framework is based on principles of Applied Behavior Analysis, Positive Behavior Support and trauma informed practice.  IHBS are available for Mass Health youth under the age of 21 with serious emotional disturbance (SED) or Autism. Youth must be enrolled in Mass Health Standard or Common Health who meet the medical necessity criteria for the service.  This service is generally HUB dependent with OPT, IHT or CSA’s as referral sources.  HUB waivers are considered on an individual basis when other CBHI services are inappropriate and must be approved by insurance.

Total Achievement Program (TAP)

TAP, or Total Achievement Program, is a therapeutic after-school program dedicated to enhancing the social and academic skills of children ages 6-12 with complex emotional and behavioral needs who have been unable to succeed in non-therapeutic community-program settings.  The Total Achievement Program (TAP) provides intensive community-based, therapeutic support for children with mental health challenges to avert psychiatric hospitalization or out-of-home placements. TAP offers individualized goal-setting, support, and services to at-risk children to help them develop the social and adaptive skills necessary to function appropriately at home, at school, and in the community.

TAP operates under a closed referral process, with referrals coming directly from the Department of Mental Health and the Department of Children and Families. Referrals can be made directly to the Department of Mental Health. Community Service Agency (CSA)

The Cape CSA is a coordination of care for youth with social, emotional or behavioral challenges (sometimes referred to as "SED" serious emotional disturbance) and their families who are involved with multiple child-serving systems (e.g., child welfare, special education, juvenile justice, mental health).  We strive to work collaboratively with the youth’s team to provide an individualized care plan that is consistent with the principles of wraparound. Intensive Care coordinators are committed to keeping youth safely in the community with the assistance of their natural supports.

Community Service Agency services are available for MassHealth youth with serious emotional disturbance (SED), under the age of 21, and enrolled in MassHealth Standard or CommonHealth who meet the medical necessity criteria for this service. Cape Cod Community Based Services (CBS)

JRI’s Cape and Islands Community Services Program collaborates with youth and families, within their own communities to create healthy relationships, teach skills and pursue opportunities that lead to independence.

JRI’s Cape and Islands Community Services Program includes:

  • In Home Therapy (funded by Masshealth)

  • Family Systems Intervention (funded by DMH)

  • Intensive Home & Community  Based Therapy (funded by DCF)

An intensive home-based support to work closely with parents and children to coach and teach practical skills and help youth learn healthy conflict resolution. JRI works with youth and families to identify and manage emotional dysregulation, behavioral issues, traumatic stress, and learning challenges, while helping to recognize strengths and foster those strengths through community activities and interventions. In-Home therapy is provided by a qualified clinician who works as part of a team that includes a Therapeutic Training & Support paraprofessional. These services may be provided in any setting where the youth is located, including the home, schools, child care centers, respite settings, and other community settings.

  • Diversion Services (funded by DCF)

The Diversion model is designed for youth ages 5-21 who are returning either to their birth, foster, or adoptive home from an out-of-home placement or have experienced multiple psychiatric hospitalizations and may be at risk for out-of-home placement in residential or acute-care settings. This model can include any number of interventions or services including Youth Mentoring, In-Home Therapy, Creative Safety Planning, Case Management and Coordination, Crisis Intervention and Stabilization (24 hours/day), Caretaker Support and Education, Recreational Services, Sibling Support, Individual, Family, and Group Therapy, and Respite Services

  • Therapeutic Mentoring (funded by Masshealth)

  • Individual Youth Support (funded by DMH)

  • Youth Mentoring (funded by DCF)

Mentoring provides one-to-one support for children and adolescents by helping them address daily living, social, and communications needs. Therapeutic mentors act like coaches who can train and guide youth in age-appropriate behavior, problem-solving, conflict resolution, and relating well to peers and adults. This service helps to ensure smooth implementation of the activities that are identified in the youth's individual care plan.  Our Mentoring services are provided to youth in any setting where the youth resides, and in other community settings such as school, child care centers, respite settings and other appropriate community settings.

  • Young Adult Peer Mentoring (funded by Masshealth, DMH &DCF)

Young Adult Peer Mentoring is a specialty expertise based on sharing one’s lived experience of mental health challenges with purpose and intent to inspire hope and motivation in a young adult who is struggling with similar concerns. Young Adult Peer Mentors use their lived experience as an active ingredient in a therapeutic process.

JRI CBS clinicians, mentors and paraprofessionals are trained in the use of evidence-based trauma-informed approaches such as the ARC framework (Attachment, Regulation and Competency) to help support and guide treatment for youth and families.

Family Networks

JRI’s Family Networks mission is to identify and secure the highest quality of supports and services for children and families involved with the Department of Children and Families in the Cape and Islands areas. Each family receives services that are tailored to their unique needs while recognizing the necessity to have diverse and culturally competent treatment providers. Interventions and provider selection are designed in a way that assists families to become self reliant, enhancing their ability to maintain success and permanency.

Do you have a question about JRI services?

Rachel has been a part of the JRI team since January, 2000. For over 20 years, Rachel has been working in the field of human services assisting families with accessing and navigating services. Rachel received her Bachelors degree in psychology and Masters Degree in Public Administration from Bridgewater State University. She was promoted in July 2005 to Family Networks Program Director where she closely worked with the Department of Children Families for 10 years ensuring that children and families received the highest quality of individualized services ranging from community based through residential care. Rachel is very dedicated to helping the individuals she works with and is committed to improving the lives of children and families. Rachel’s passion for creative service programming inspires her in her role as JRI Service Navigator.
